Do these lines end fight?
Do Ocean like words 'We'
split into rivers 'You' and 'Me' ?
Before, Our words were different, but they implied same feelings
But now, The same single word has got several meanings
At last,
Feelings changed, Memories remained
So close to each other we stay, Yet our hearts go in parallel way!

Seriously, the poem is very good, the thought, the rhyming, everything is perfect. I'm sorry, can't suggest a title as I think you can feel the poem better as it's your baby...
